Each member of the household will be asked whether he/she experienced any illness over the last 2 weeks. If so, the type of sickness and the most recent action taken should be recorded using the given codes. Some people may not want to declare that they, or their children were sick. So be careful in asking whether each member of the household was sick in the past 14 days. You may have to use phrases such as “were you unable to work on any day in the past 14 days?” If “yes” ask the type of sickness. Select the correct answer from the codes in the footnotes to the questionnaire.
