This questionnaire seeks information on incomes earned by individuals in the household from all sources. Contributor number in Column 14-15 should correspond with the WMSI/94 serial number. Responses to each of the column headings provide a different source of income. Note that some of the incomes will be reported on monthly basis while others on an annual basis in last 12 months.
